- A nurse supervisor will visit your home to complete an initial assessment and the required paperwork. Documentations will include consent for treatment, development of a Plan of Care with your input, Medication Profile, Emergency/disaster Plan, staff schedule, and any other information that needs to be prepared.
- The supervisor will assist your family in settling in if the patient was discharged from the hospital before our admission. The staff will ensure that the child has an emergency go-bag and extra supplies for emergency care.
- Your assigned nurse will arrive as scheduled to provide hourly care, communicate with your physician as needed, educate the family of care, and assist with other medical needs.
- Your assigned nurse will document care using our point-of-care device, where available, and we will need you to sign off on the care notes.
- The scheduler from our office will contact you, provide you with a monthly nursing schedule, and communicate any changes as applicable.
